The Patient Centered Medical Home

Heritage Victor Valley Medical Group has launched its new Patient-Centered Medical Home program. This will bring to Heritage patients the opportunity for a new level of personal health through increased patient contact, quick response and fully coordinated care from Heritage.

Barstow, CA – Heritage Victor Valley Medical Group has initiated its Patient-Centered Medical Home program in Barstow. However, starting with the primary care physicians and their care partners working in the community the program is creating meaningful connections between providers, patients, and families across the high desert region.   

Christina Brown, L.V.N. is part of the Heritage team delivering these services in the town of Barstow.  She is working with all the Heritage primary care physicians and the needs of their Heritage members. “The Patient-Centered Medical Home program allows me to work closely with my patients. In addition, providers building a relationship that reaps tremendous results that I enjoy being a part of”.    

Team-based coordinated care

Is learning about a patient’s family, their surroundings, their short-term and long-term care needs. This includes vaccinations, test reminders, and notification of results. This patient relationship based program is dedicated to the needs of the patients.  Our goal is to improve their care for longer. Above all, we aim to give them healthier lives from their primary care physicians.

In conclusion, this location was pioneered in 1967 by the American Academy of Pediatrics for children with specialized health needs.  With today’s increasingly complicated healthcare system the medical community has expanded this term to include all children and adults whether their medical needs are simple or complicated providing them with a team including but not limited to their primary care physician, nurses and specialists to provide all the assistance and support they need to meet their medical needs from their medical home location.
