What to Expect at Your Senior Wellness Visit

Lab Tests

When you schedule your Wellness Clinic visit, we will order the following lab work:

  • CBC panel – checks white and red blood cells.

  • CMP test – checks liver and kidney function.

  • Hemoglobin A1c – screens for diabetes.

  • Lipid Panel

  • PTH

  • Thyroid Panel

  • Urine test.

  • Vitamin D and B12 levels.

At Your Appointment

  • Your appointment will last about one hour.

  • You’ll meet one-on-one with our knowledgeable staff.

  • Count on us to listen, answer questions, and recommend services that are right for you.

Your Physical, Mental & Emotional Well-being

When you arrive, a member of your care team will:

  • Ask about your current health and any symptoms (such as balance issues).

  • Check blood pressure, pulse, respiration rate, and temperature.

  • Complete memory tests and dementia screening.

  • Measure height and weight.

  • Screen for signs of depression and anxiety.

  • Review any suspicious skin lesions.

Test Results & Personalized Guidance

During the second half of your visit, your NP or PA will:

  • Explain your test results.

  • Share tips for illness and injury prevention.

  • Talk through your health goals.

  • Provide personalized care and advice, based on a review of your medical records and health history.

Vaccinations

  • Receive your flu shot if you’re due.

  • Receive a pneumonia (pneumococcal) shot if recommended for your age or health status.

  • Conveniently get both during your Wellness Clinic visit, so you can stay protected without extra appointments.

Orders for Screening Tests (when needed)

During your Wellness Clinic visit, you may also receive orders for:

  • Chest X-Ray.

  • Breast & Colon Cancer screening.

  • Bone Density test.

  • Breathing test.

Follow-Up & Annual Care 

  • Your Wellness Clinic team will send a complete report of your visit to your primary care provider, ensuring your care is coordinated and effective.

  • As a Wellness Clinic patient, you’ll also receive a yearly reminder call from HVVMG to schedule your next visit—helping you stay on top of your health while focusing on family, friends, and enjoying life.


Meet Your Wellness Providers 

Elsa Uresti FNP-C

Elsa is a Family Nurse Practitioner with over 16 years of experience in nursing. She earned her Post-Master’s in Family Nurse Practitioner from West Coast University in July 2023 and also holds a Master’s in Nursing Education from California State University, Dominguez Hills. Since December 2023, she has been practicing as an NP, including a year and a half of specialized experience in pulmonology under the guidance of Dr. Sahai.

She is passionate about preventive care, emphasizing the importance of yearly wellness checkups to identify potential health risks early through screenings. By detecting issues sooner, patients have better outcomes and can often avoid more serious complications. Dedicated to patient education, she focuses on empowering individuals with the knowledge and tools needed to manage their health conditions, prevent complications, and improve their overall quality of life.

Outside of her professional role, she enjoys traveling with family and friends and has a love for baking sourdough bread. Heritage is proud to have her as part of our Wellness Team.


Carol Lam, PA-C


Carol is a Physician Assistant with over 21 years of experience in healthcare. After earning her degree in Physician Assistant Studies from Western University, she spent more than 17 years working in emergency rooms, hospitals, and urgent care facilities before transitioning into family practice.

For the past several years, Carol has served as a provider at HVVMG Wellness Center, where she primarily cares for senior patients. With a strong passion for preventive medicine, she values the opportunity to build meaningful relationships, learn patients’ life stories, and track their health progress through yearly wellness checkups. She finds it rewarding to see patients return each year, demonstrating their commitment to proactive healthcare—a refreshing shift compared to her earlier years in the ER.

Outside of medicine, Carol enjoys dancing and teaching yoga. She is an amateur ballroom dancer who occasionally competes, and she also takes ballet, contemporary, and hip-hop classes when her schedule allows. Beyond the studio, she loves spending time outdoors with her two dogs, whether hiking or visiting the dog beach, and cherishes family vacations and gatherings.

Carol is proud to be part of Heritage, where she enjoys serving the senior community and supporting their health and wellness with compassion and care.


Brandi Funk, FNP-C

Brandi Funk is a board-certified Family Nurse Practitioner with more than 20 years of healthcare experience spanning geriatrics, pediatrics, primary care, and preventative health. Her clinical interests include preventative medicine, addressing healthcare disparities, and promoting health education.

Brandi earned her undergraduate degree in nursing in 1993, followed by a master’s degree in nursing in 1998, and later a Post-Master’s Certification as a Family Nurse Practitioner in 2006, all from Azusa Pacific University in Azusa, California. Before becoming a nurse practitioner, she served at City of Hope Medical Center in Duarte, CA for over eight years, where she gained invaluable experience caring for patients in a wide range of clinical settings.

For the past decade, Brandi has specialized in performing Annual Wellness Exams—beginning at Desert Oasis Healthcare in 2015 and continuing most recently at HVVMG in 2024. She believes the cornerstone of these visits is early detection through health screenings and fostering meaningful connections with patients. Her approach emphasizes listening, compassion, and collaboration to help patients reach their personal health goals and achieve optimal well-being.

Brandi finds great fulfillment in working with the senior population, valuing their stories, life experiences, and the opportunity to support their health through education and partnership.

A Southern California native, Brandi is proud to serve her local community. Outside of work, she enjoys spending time at the beach, listening to podcasts, attending concerts, and exploring new vegan restaurants. She also loves quality time with her husband, two sons, and their cats, Mochi and Muffin.
