Fall Risk Prevention ProgramOur Fall Risk Prevention Program provides comprehensive safety assessments, education, and training to help members prevent falls and maintain independence. The program follows the CDC STEADI guidelines, which are proven to reduce fall risk in seniors.
You can join the program through a referral from your healthcare provider or by self-referring.
How it works:
Initial Appointment: A 60-minute visit at the Wellness Clinic to assess your needs and create a personalized fall-prevention plan.
Follow-Up: A registered nurse will call you two weeks later to check on your progress.
Discharge Evaluation: Three months after your initial visit, you’ll return for a 20-minute in-office evaluation to measure your progress and determine how helpful the program has been.
