Why Fall Prevention Matters

According to the Centers for Disease Control and Prevention (CDC), millions of adults age 65+ experience falls each year. Falls can lead to broken bones, hospital stays, and loss of independence.

The good news? Many falls are preventable.

Fall Risk Prevention Program

At Heritage Victor Valley Medical Group, our Fall Risk Prevention Program is designed to help you stay safe, steady, and independent. Call the Wellness Department @760.261.6422 to Schedule

Step 1: Initial Wellness Visit

Your journey begins with a 60-minute appointment at our Wellness Clinic. During this visit, we will:

  • Assess your fall risk factors
  • Review medications
  • Evaluate balance and strength
  • Provide education and personalized recommendations

Step 2: Follow-Up Support

Two weeks after your initial visit, one of our Registered Nurses will call you to:

  • Check on your progress
  • Answer questions
  • Reinforce safety strategies

Step 3: Discharge Evaluation

Three months later, you’ll return for a 20-minute in-office evaluation to:

  • Measure improvements
  • Review your progress
  • Determine how the program has helped reduce your fall risk

Who Can Participate?

Patients may join the program:

  • By referral from their healthcare provider
  • Through self-referral

Meet Our RN Health Educator

Hello, my name is Kathleen, and I’m the Health Nurse Educator at Heritage Victor Valley Medical Group. I’ve been a nurse for many years and have had the privilege of working in several areas of healthcare, including psychiatric care, emergency services, and correctional health.

After retiring in 2021 to spend more time with my grandchildren, I decided to return to nursing in 2023 to join the Heritage team. Today, I help lead our Fall Risk Prevention Program and provide health education through classes, staff training, and our Path to Vitality lifestyle program.

I’m passionate about helping patients stay healthy, safe, and informed so they can live their best lives. When I’m not working, I enjoy cooking, knitting, gardening, and spending time with my family and my three rescue dogs.
