Perdiem and Full Time Position Open- Graduate from an accredited school for Licensed Vocational Nurses.
- Current California Licensure, CPR certification.
- At least one-year acute hospital experience, if applicable.
- ACLS certification is required within 6 months of employment.
- Good computer skills a must, 35-40 WPM.
- Ability to work in a multi-task high stress environment.
- Obtain complete and accurate vital signs and assist patients to exam room.
- Anticipate, assist and perform procedures ordered by the provider.
- Maintain timely patient flow and room availability.
- Reinforce patient education and participate in patient teaching.
- Participate in unit duties such as cleaning and sterilizing instruments and ordering supplies.
- Maintain a current, accurate pathology and lab log.
- Maintain and monitor all equipment used for patient care and testing.

- LVN Urgent Care - Victorville
- Actively participate in nursing/lab QA by identifying problems, providing solutions and evaluating outcomes.
- Ensure appropriate handling and accurate documentation of all medications ordered by the provider.
- Knowledgeable with referral system and ensures accurate and timely follow through with all referrals generated.
- Competent with all aspects of patient care provided in each department thus able to float to all departments if necessary.
- Knowledge of HVVMG computers as pertains to job.
- Universal precautions utilized in all patient care.
- Supervise patient care performed by Medical Assistant and LVN.
- Assume responsibilities for narcotic medications as assigned.
- Responsible for blood withdrawal and IV therapy, including administration of medications.

Director of Case Management - Victorville
This is a Full Time PositionResponsible for the management and oversight of Inpatient Acute case management and concurrent review, SNF Case Management as well as the Complex Case Management programs including High Risk case management, disease management, palliative care and hospice. The position directly supervised the Manager, Case Management and the Manager, Care Coordination. Responsible for the development of ambulatory case management, inpatient concurrent review, and disease management programs that maximizes quality while utilizing appropriate resources within the members' benefit plans and established contracts.
- Bachelors of Science in Nursing (B.S.N.) or related field.
- Current CPR certification.
- Good working knowledge of managed care (health plan/insurance and medical group aspects).
- Comprehensive background in utilization review, case management, prior authorization, referral processing and quality management.
- Understand contracting, subcontracting and its correlation to cost of health care in varying delivery systems, e.g. staff models, IPA and group models.
- Good working knowledge of HCFA, NCQA, Interqual, M&R standards and regulations.
- Minimum five years in acute care/clinical setting.
- Minimum three years of utilization management experience (review/discharge planning/case management).
- Minimum two years of prior management/supervisory experience.
- Excellent written and verbal communication skills.
Nurse Case Manager - Victorville
This is a full-time position.Responsible for evaluating patient and medical records using Interqual and Milliman guidelines to monitor SI/IS, length of stay criteria and facilitate discharge planning and individual needs to facilitate a smooth transition to a lower level of care/home discharge with recommendations for continued care for optimum management.
- Graduate from an accredited Licensed Vocational Nursing Program.
- Current California License.
- Minimum of 2 years clinical nursing experience.
- Minimum of 1 year utilization review experience.
- Ability to work in a multi-task, busy environment.
- Current CPR.
- Good computer skills beyond basic application.
- Perform daily onsite inpatient concurrent reviews, telephonic daily reviews for out of area inpatients as needed. All medical records will be document in the electronic Medical Records (EMR) within the same day with completion prior to the following daily case management 9:00 meeting.
- Daily telephonic skilled nursing concurrent reviews with weekly onsite visits as needed applies to the same EMR documentation requirements a inpatient.
- Review medical records of each patient and be prepared to discuss and give member updates, care plan, discharge plan, barriers during daily Care Team Case Management meetings.
- Real time daily communication with HVVMG hospitalist and HVVMG Medical Director regarding utilization, discharge plans and issues if needed. Telephonic daily communication with attending physician and case management for inpatients admitted to out of area hospitals and skilled nursing facilities.
- Admission inpatient assessment in a timely manner to identify barriers, concerns, needed referrals, discharge plan.
- Case Management works closely with Quality Improvement. All readmissions, issues, concerns, will be documented in the PQI form for reporting/tracking purposes.
- Need to be crossed trained and multi skilled in all catastrophic case management programs.
Network Administrator - Victorville
This is a full-time position.Under the direct supervision of the Director of MIS the Network Administrator maintains and supports the efficient operation of the HVVMG local-area network and systems critical to the business operations of HVVMG; provides 1st level technical support to HVVMG-MIS groups; identifies, recommends, and implements new technology to support increased efficiency of HVVMG operations.
- BS in Computer Science.
- 3-5 plus years working with hardware, systems and software for network infrastructure, design, implementation, support and strong working knowledge of IP internetworking, LAN switching, WAN and network security.
- Experience with Microsoft Windows networking, TCP/IP, Ethernet, Cisco Router configuration a plus.
- Microsoft server 2010.
- Microsoft SQL, MSExchange, Microsoft IIS.
- SAN knowledge, router, firewalls, remote access.
- Assist in the design, integration and implementation of network development.
- Perform routine maintenance and repair to network and system to maintain optimal operation and maximum up time. Monitor and tune the HVVMG network regularly.
- Provide immediate response to down systems. Participate in the —On Call˜ program to provide support for down systems during off hours.
- Develop and maintain Disaster Recovery Plan for network and systems. Provide 1st level technical support to assist MIS technical staff in accomplishing their routine tasks. Provide written instruction on all processes necessary to add, change, and remove standard equipment (computers, terminals, printers) as well as basic troubleshooting techniques.
- Support and provide recommendations for the purchase and installation of new or additional equipment and software.
- Identify, recommend, and manage the implementation of new systems and technology as well as enhancements to existing systems and technology to improve the overall efficiency and effectiveness of the organization.
- Implement and verify that all automated procedures are working properly; specifically automated backups, server startup and shutdown scripts, print and communication devices.
- Provide training and instruction to MIS technical support staff to increase their technical knowledge and value to the organization.
- Present and maintain a professional image of the HVVMG Departments.
- Maintain vendor relationships to assist in obtaining timely support for systems and network problems.

IS Technical Support - Victorville
This is a full-time position.Under the direction and supervision of the Information Technology Manager, provide 1st level technical support to the Heritage Victor Valley Medical Group in Victorville.
- Associates Degree in computer science or equivalent experience. Bachelor's Degree preferred.
- Good Written and verbal skills.

- Prior user support and/or training experience.
- Prior experience using or supporting MEDIC + software.
- Prior experience using NextGen or EMR software.

- Develop an expert knowledge of Heritage Medical Group new and existing computer software and equipment.
- Create HVVMG specific documentation and procedures to support the proper use of the computer systems.
- Train existing staff on the proper use of new and existing computer programs and equipment.
